What is hair?
Hair, part of the family of phaneres (hair, nails), is primarily composed of keratin and consists of the visible hair shaft and the bulb under the scalp. At the base of the bulb is the dermal papilla, the biological engine. The hair follicle, or root, includes the bulb and epithelial sheaths. The inner epithelial sheath surrounds the shaft, aiding growth, while the outer contains stem cells for the hair cycle. Intense division of keratinocytes in the bulb produces the shaft and sheaths. The papilla, rich in fibroblasts, facilitates oxygenation and nourishment of the bulb, regulating its biological functions through its vascularization and collagen production by fibroblasts, essential for hair anchoring.
The hair life cycle
The hair life cycle includes three main phases: anagen (growth), catagen (rest), and telogen (shedding), each renewing several times over an individual’s lifetime asynchronously. “Telogen effluvium” refers to reactive hair loss due to an imbalance in this cycle.
Anagen: Active growth phase initiated in the dermal papilla, characterized by intense cellular division forming the hair follicle and its sheaths. The hair shaft is keratinized, and the follicle develops deeply to nourish the hair, lasting 2 to 5 years. Catagen: Transition phase where growth stops, lasting about 3 weeks. Telogen: Final phase where the hair does not grow but remains in place until it falls out, allowing a new cycle to begin with a new follicle in the anagen phase.
